Optimise the system of work for the majority of cases rather than the exceptions

Description
Ensure that organisational systems, services, processes, policies and rules are tuned to effectively and efficiently support the most common scenarios. Avoid adding unnecessary impedance and delay to these common scenarios with guardrails to catch exceptions.
Start with the most simple solution and validate if exception scenarios occur, rather than second-guess with additional processes, policies and rules.
Rationale
Many processes are encumbered and complicated with additional processes to support rare scenarios. The result is added latency and waste that impedes value creation and reduces levels of trust and motivation.
